Description DATE: 11/8/2021 Pay rate is dependent upon number of years with valid CNA certification. Essential Functions To provide personal aspect of care for patients with the goal of increasing their level of comfort and maintaining personal hygiene. Work under clinical supervision of the Clinical Support Supervisor and the Registered Nurse. Carry out patient scheduling provided by the Staffing Specialist or assigned designee. Duties may be adjusted at the discretion of the nurse based on the evaluation of the patient's needs and the ability of the CNA.

Challenges in Position Deliver personal care in varied home and facility settings. Rotating weekend and holiday work required. Distant driving required. Frequently deals with different emotions expressed by families or caregivers related to impending death or death of loved ones.

Education and Experience High School Diploma or GED required
Certified Nursing Assistant certificate from college, technical school or equivalent training program, and minimum one year recent experience as a Certified Nursing Assistant is required.
Certificates, Licenses, Registrations, Vaccines Valid COVID-19 vaccine required
Influenza vaccine during influenza season required
Current CNA certification as required by the state of Georgia or Alabama
Current CPR BLS certification required
Valid drivers license, reliable transportation, and auto insurance coverage per policy.
Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Reasoning Ability Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.
Ability to deal with problems involving a few concrete variables in standardized situations.
Language Skills Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence.

Position Requirements (not all-inclusive) Provide personal hygiene and comfort measures to the patient.
Take blood pressure, temperature, pulse, and respirations of the patient during each visit with the patient, or per plan of care.
Ability to transfer patient, e.g., bed to chair, bed to commode, chair/commode to bed.
Assist with ambulation and exercise of patient, as per plan of care.
Change patient bed linens.
Assist with feeding of patient and/or preparing patient a light meal per plan of care.
Perform bowel care, e.g., documenting last bowel movement and notifying primary nurse of no bowel movement within 2 3 days.
Deliver medical supplies to patient homes.
Perform light housework, e.g., empty patient trash, vacuuming and/or sweeping patient living area, cleaning up after patient bath, washing of patient clothes, and/or anything stipulated in the patient's plan of care.
Follow individualized care plan for each patient.
Maintain records for patient charts on all care provided.
Report changes in patient's condition or home environment to Registered Nurse for follow through and appropriate intervention.
Is aware of emotional status of patient/family and demonstrates willingness to understand the emotional problems associated with the illness.
Take all reasonable precautions to maintain privacy and non-disclosure of any confidential information about patients, other staff members, and the general operations of Columbus Hospice.
Take weekend/holiday call, on a rotational basis, as assigned by staffing coordinator or assigned designee.
Participate in hospice training as required by policy, to include 15 hours of CEUs annually, of which 12 hours must be Columbus Hospice specific. Competency check per policy.
Attend one memorial service per evaluation year.
Other duties as assigned and appropriate for level of practice. (Example: may be taught simple wound care).
