DESCRIPTION

About the Position

Lane County Behavioral Health is looking for a dynamic individual to fill the role of Community Health Nurse working in collaboration with our child, adolescent and adult mental health services teams. This exciting position provides support work to severely and persistently mentally ill individuals in a clinic based setting.

Duties include, but are not limited to providing nurse case management and care management for individuals experiencing severe and persistent mental illness with co-morbid medical conditions; triage and service coordination with clinical and medical staff, primary care providers and allied community agencies, including crisis intervention as required; health and wellness promotion through identification of mental health issues and intervention/prevention of health problems, skills training and patient and family psycho-education; demonstrate competency in consulting with primary care providers, psychiatry, emergency services and other services on behalf of mutual patients.

Lane County Behavioral Health provides services to more than 1,600 individuals with behavioral health conditions. We work with community partners to provide a continuum of care from prevention through treatment and aftercare in an environment that promotes a culture of resilience and recovery.

QUALIFICATIONS

Training & Experience:
  • Bachelor of Science in Nursing degree from an accredited college or university.
Any equivalent combination of experience and training that provides the required knowledge and abilities is qualifying.

If you enroll in the HDHP the County will contribute to your HSA: $1,500 (individual) or $3,000 (family) annually. This amount is prorated based on date of hire.
