NOW HIRING: MDS Coordinator

MDS Coordinator Responsibilities Include:

• Conduct MDS assessments in accordance with regulatory guidelines and facility policies.
• Ensure accuracy and completeness of resident assessments.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Monitor resident progress and update care plans as necessary.
• Ensure compliance with state and federal regulations, including CMS guidelines.
• Assist with Quality Assurance and Performance Improvement (QAPI) initiatives.
• Provide guidance and training to staff regarding MDS processes and documentation.

MDS Coordinator Qualifications:

• Registered Nurse (RN) with current NJ state license.
• Experience in MDS coordination or a related field.
• Strong knowledge of MDS assessments, Resident Assessment Instrument (RAI) process, and regulatory guidelines.
• Excellent organizational, communication, and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
