Medicare Wellness LPN/LVN

$20.00-$27.00

Overview/ Position Summary:

The Medicare Wellness LPN/LVN performs Medicare Wellness visits with identified patients in person or via telehealth, and collaborates with providers for documentation, review, and sign off completion. The ideal candidate for this position must possess compassion, participate in active collaboration and communication, and must perform appropriate clinical judgement in patient care. The RN must be educated on chronic disease management and preventative care, providing related teaching to patients as needed. They will conduct patient charts reviews for pre-visit planning and other clinical requests, and are responsible for conducting outreach, scheduling and verification activities for preventative services and Medicare Annual Wellness visits.

Summary Of Clinical Duties:

• Reports to work on time and prepared to begin working at the start of shift
• Conducts thorough and respectful communications with providers
• Reviews Medicare lists/daily schedules of providers to pull MWV
• Relays patient concerns to the appropriate providers during the wellness visits
• Schedule Medicare Wellness visits via Zoom, Telemedicine, or in-clinic
• Completes Medicare wellness exams with the assistance of clinical assistant, including but not limited to vitals, eye exams, hearing exams, EKG, and other task as required
• Mails NM MOST to patients
• Completes additional administrative duties, including but not limited to faxing, scanning, entering referrals, ordering labs, and communication by phone with patients
• Helps with triaged chronic care management and remote patient monitoring patients as needed
• other duties as assigned

Education and Experience:

• Current NM licensed LPN/LVN
• Two years of experience in primary healthcare or related setting
• CPR certification required

Work Environment:

• Frequent interaction with a diverse population, including team members, providers, patients, insurance companies, and other members of the public
• Work hours subject to office schedule to ensure coverage during hours of operation
• Participate in regular staff meeting and quality improvement projects to improve patient care
• Travel to alternate provider offices to perform MWV/ Occasional travel to attend meeting or trainings
