JOB DESCRIPTION: CASE MANAGER

Covenant House CA (CHC) is excited to announce the launch of its new Safe Haven Housing Program in Anaheim, CA, for young people experiencing homelessness and human trafficking. The staff at CHC Anaheim will be part of a unique experience of being the first to work at the site, becoming a critical part of CHC’s history. Drawing upon CHC’s 30+ years of experience serving young people, ages 18-24, staff will assist to develop a program that effectively meets the unique needs of young people experiencing homelessness and human trafficking in Orange County. POSITION SUMMARY:
CHC is looking for passionate, highly skilled and energetic Case Managers to support the launch of its new Safe Haven Housing Program. The Case Manager will possess excellent communication and organizational skills and provide support to young people that is strength-based, trauma-informed and harm reduction focused. The Case Manager will model our mission of providing absolute respect and unconditional love to all young people and will value working within a multi-disciplinary team to create a safe inclusive environment for all.
Under the supervision of the Director of Orange County Programs, the Case Manager is responsible for the on-going engagement, assessment and service planning with young people to meet their housing, health and wellness, employment and educational goals, as they move towards permanent housing. Other functions include crisis intervention and de-escalation, documentation in multiple databases, facilitation of psycho-educational and support group activities, community engagement and networking with local service providers.
SCHEDULE: Full-Time, 40 hours per week
SALARY RANGE: $23.50 /hour
ESSENTIAL FUNCTIONS:
1. Provide Case Management services with the goal of developing positive relationships with young people through the utilization of the case management system, including:
Intake:
• Welcome young people to the program, ensure they feel comfortable, safe and understand the program structure, opportunities for engagement and access to supplies that they might need.
• Review expanded intake documentation and ensure it is entered into ETO and HMIS upon entry into the program.
Full Assessment:
• Review and integrate the Initial Background Interview (IBI) into the Individual Service Plan.
Individual Service Plan:
• Develop a Success Plan with each young person upon completion of assessment.
• Revise and update the Success Plan as necessary. An updated plan is written, at minimum, after two weeks of intake, and upon the monthly meeting that occurs with the young person and their identified circle of support.
• Maintain a calendar of the young people’s work schedule, appointments, etc., in conjunction with their Success Plan.
• Develop and maintain knowledge of and connection to community resources.
• Make appropriate referrals for services within CHC and to community agencies.
• Engage in positive transition planning with the young people.
• Monitor young people’s progress of self-identified goals and coach young people to plan, implement, and follow-through with tasks.
• Motivate young people to improve wellness including physical and mental health, substance use, and other areas that will improve their overall functioning.
Documentation:
• Document regular weekly progress note updates in young people’s case records (HMIS and ETO).
• Document pertinent information in Transition Log, Critical Incident Reports, Follow-Up forms etc.
Case Review:
• Prepare and present assigned cases at weekly case review meetings and facilitate monthly Success Planning meetings.
Group Facilitator:
• Facilitate psycho-educational and support group activities with young people on a regular basis.
2. Perform duties to meet the needs of young people while ensuring adherence to the daily program schedule. This includes, but not limited to:
• Facilitate case management meetings with young people and young people’s support network.
• Assist with upholding the cleanliness and safety of the facility.
• Provide crisis de-escalation and intervention as necessary.
• Provide regular supervision of residents and activities when appropriate or asked.
• Support outreach and engagement efforts to identify and support move in of young people from the street to shelter.
3. Maintain daily communication between case management and residential staff for seamless coordination and best practice collaborations.
4. Provide unconditional positive regard to young people regardless of behavior, race, class, culture, country of origin, disability, self-identified gender, sexual orientation, gender expression, immigration status, language capacity or ability to complete case plan requirements.
5. Attend weekly supervision with supervisor and weekly meetings as assigned.
6. All other duties as assigned.
JOB REQUIREMENTS:
EDUCATION: Bachelor’s degree in Psychology, Sociology, Social Work or related field OR an Associate degree in Psychology, Sociology, Social Work or related field OR High School Diploma with a minimum of 5 years case management experience.
EXPERIENCE: 2-4 years case management experience, preferably with young people ages 18-24 experiencing homelessness or human trafficking, identify as LGBTQ, are gender non-conforming and/or who have lived expertise. Knowledge of public benefits and community resources in Orange County and the Greater LA/Orange County Area. Working within a strength’s based, harm reduction and trauma informed framework.
SKILLS: Must be a self-starter and team player with the ability to work well with others. Knowledge of adolescent development and ability to build positive rapport with young people. Excellent communication skills as demonstrated by concise, timely and professional correspondence, both written and verbal. Bilingual in Spanish preferred but not required.
TRANSPORTATION: Must have a valid CA driver’s license and good driving record. May need to transport clients/staff if required using agency transportation.
